Pharmacological properties
Domperidona este un agent prokinetic si antiemetic, antagonist dopaminergic (D2), care actioneaza predominant periferic.
Proprietati principale:
- Antagonist al receptorilor dopaminergici D2 la nivel gastrointestinal → creste motilitatea gastrica si accelereaza golirea stomacului.
- Are efect antiemetic prin actiune asupra zonei trigger a varsaturilor (CTZ), fara a traversa semnificativ bariera hemato-encefalica.
- Spre deosebire de alte antiemetice, are mai putine efecte sedative si extrapiramidale.
- Stimuleaza peristaltismul gastric si reduce refluxul gastroesofagian asociat distensiei gastrice.
Warnings and precautions

- Se administreaza, de regula, cu 15–30 minute inainte de mese.
- Nu se recomanda utilizarea la pacienti cu: afecțiuni cardiace preexistente (aritmii, prelungirea intervalului QT), insuficienta hepatica, tulburari electrolitice severe (hipokaliemie, hipomagneziemie).
- Poate creste riscul de aritmii ventriculare la doze mari sau in combinatie cu medicamente care prelungesc QT-ul.
- Se utilizeaza cu prudenta la varstnici.
- Nu se administreaza concomitent cu inhibitori puternici ai CYP3A4 (creste riscul de aritmii).
- Reactii adverse posibile: gura uscata, dureri de cap, ameteala, rar tulburari de ritm cardiac.
